Tooth decay is a highly destructive dental disease, responsible along with periodontal (gum) disease for most adult tooth loss. And we become even more susceptible to it as we get older.
One form of decay that’s especially prominent among senior adults is a root cavity. Similar to a cavity in the crown (visible tooth), this form instead occurs at or below the gum line in the roots. They happen mainly because the roots have become exposed due to gum recession, a common consequence of periodontal (gum) disease and/or brushing too hard.
Exposed roots are extremely vulnerable to disease because they don’t have the benefit of protective enamel like the tooth crown, covered instead with a thin and less protective mineral-like material called cementum. Normally, that’s not a problem because the gums that would normally cover them offer the bulk of the protection. But with the gums receded, the roots must depend on the less-effective cementum for protection against disease.
Although we treat root cavities in a similar way to those in the crown by removing decayed structure and then filling them, there’s often an added difficulty in accessing them below the gum line. Because of its location we may need to surgically enter through the gums to reach the cavity. This can increase the effort and expense to treat them.
It’s best then to prevent them if at all possible. This means practicing daily brushing and flossing to remove bacterial plaque, the thin, built-up biofilm on teeth most responsible for both tooth decay and gum disease. You should also visit your dentist at least twice a year for professional cleanings and advanced prevention methods like topical fluoride to strengthen any at-risk teeth.
You should also seek immediate treatment at the first sign of gum disease to help prevent gum recession. Even if it has occurred, treating the overall disease could help renew gum attachment. We may also need to support tissue regeneration with grafting surgery.
Root cavities are a serious matter that could lead to tooth loss. But by practicing prevention and getting prompt treatment for any dental disease, you can stop them from destroying your smile.

Periodontal disease may start in the gums’ superficial layers, but it’s not likely to stay there. As the disease moves deeper it can wreak havoc on tooth roots and bone as well as gum tissue attachments. Teeth with multiple roots are in particular peril because of the “forks” called furcations that form where the roots separate from each other. Infected furcations can be very difficult to treat.
We primarily treat gum disease by removing its main source, a thin film of bacteria and food particles called dental plaque that builds up on teeth. To remove it we most often use special hand tools or ultrasonic equipment to vibrate it loose. As the plaque and tartar diminish, the infection begins to wane.
But we can’t be completely successful in stopping the disease if any lingering plaque deposits remain. This especially includes furcations where the infection can cause significant damage to the roots. Although cleaning furcations of plaque can be difficult, it’s not impossible with the aforementioned tools and antimicrobial substances to disinfect the area.
The real problem, though, is access—effectively getting to the furcations to treat them. We may need to perform a surgical procedure called flap surgery where we create a hinged flap in the gum tissue to move it aside and access the root area beneath. Afterward we replace the flap and suture the tissue back in place.
In some cases, the infection may have already caused significant damage to the tissue and underlying bone. We may therefore need to graft gum or bone tissues to these damaged areas to stimulate re-growth. We may also need to surgically reshape the gum attachments around a tooth to make it easier in the future to access and clean the area.
These additional treatments around furcations can be very involved and labor-intensive. That’s why the best outcomes occur if we’re able to start treatment in the early stages of an infection. So, if you notice red, swollen or bleeding gums contact your dentist as soon as possible. Treating gum disease as early as possible will help ensure your tooth roots won’t suffer extensive damage.

September is National Gum Care Month, an ideal time to talk about how to keep your gums healthy. Unfortunately, nearly half of adults have gum disease, which can damage the soft tissue and bone supporting the teeth. In fact, advanced gum disease is the number one reason for tooth loss among adults, and it’s associated with other health problems, including cardiovascular disease, diabetes and pregnancy complications. But there’s good news—gum disease is preventable and, in its early stages, even reversible. Here are some tips for taking care of your gums:
Keep up a good oral hygiene routine
Gum disease begins with plaque buildup, so attack the plaque in your mouth with good dental hygiene. Spend two minutes morning and night brushing all surfaces of your teeth, and floss once a day to get rid of plaque that forms between teeth.
Use a soft toothbrush
The American Dental Association recommends brushing gently with a soft toothbrush. Hard bristles can damage your gums and cause them to pull away from the teeth.
Swish with a mouth rinse
Consider using a mouth rinse. Over-the-counter and prescription oral rinses are available to help wash away food debris, reduce plaque and fight gum inflammation.
Say no to tobacco
Smoking is one of the biggest risk factors for developing gum disease. And it’s not just cigarettes but all forms of tobacco, including e-cigarettes, pipes and chewing tobacco, that raise your risk of gum disease.
Eat a healthy diet
For the best gum health, avoid refined carbohydrates (sugary and starchy foods) and make sure you are getting enough vitamin C, vitamin D and antioxidants (found in berries and green tea, for example). In addition, studies suggest that a diet rich in omega-3 fatty acids may reduce your risk of gum disease and other inflammatory conditions. These healthy fats are found in many fish, nuts, seeds, soy products and vegetable oils.
Come in for regular cleanings
Professional dental cleanings can remove plaque from the tiny spaces that are difficult to reach by simply brushing and flossing. And once plaque hardens to form calculus (tartar), it cannot be removed during your regular oral health care routine at home. Further, at the dental office we can detect gum disease in its early stages—and the earlier gum problems are caught, the more easily they can be reversed.
A loose permanent tooth is not a good thing—and not something you should put off having examined. That’s because a loose tooth could soon become a lost tooth.
How we treat it depends on its underlying cause, which could be one of two types. One is primary occlusal trauma, meaning the affected tooth has experienced accidental trauma or higher biting forces than it normally encounters. This usually happens because of teeth grinding habits.
It could also be secondary occlusal trauma. Unlike primary trauma where the supporting gums and bone may be reasonably healthy, secondary trauma occurs because these structures have been severely damaged by periodontal (gum) disease. As the gums begin to detach from a tooth and its underlying bone deteriorates, even normal biting forces can loosen it.
If gum disease is present, our first priority is to bring it under control. We do this primarily by removing all dental plaque (a thin film of bacteria and food particles that triggers the infection and sustains it) and calculus or tartar (calcified plaque). This can take several sessions and, in the case of deep infection, may require a surgical procedure.
On the other hand, if teeth grinding is the primary cause, we’ll focus on minimizing the habit and its effects. One way is to create a custom-fitted guard worn to prevent upper and lower teeth from making solid contact. You may also need to improve your management of stress—another factor in teeth grinding—through medication, therapy or biofeedback.
In either case, improved periodontal health will help the gums naturally regain their strong attachment with help, if necessary, from gum tissue or bone grafting surgery. But this healing process can take time, so we may need to secure a loose tooth in the interim by splinting it to neighboring stable teeth. This usually requires bonding rigid material or metal across the back of all involved teeth or in a channel cut along the teeth’s biting surfaces. In this way the more stable teeth support the loose one.
Splinting may be temporary as the mouth heals from disease or trauma and the teeth regain their stability. In some cases, though, it may be permanent. Either way, dealing promptly with a loose tooth can help ensure it’ll survive—so see your dentist as soon as possible.

Is there a link between periodontal (gum) disease and cardiovascular disease? Medical researchers are endeavoring to answer this intriguing question, but early findings seem to say yes. If it bears true, the findings could advance treatment for both diseases.
There is one thing that can be said for certain: inflammation is a factor in both diseases’ progression. Gum disease begins as an infection caused by bacteria growing in plaque, which is made up of bacteria and a thin film of food remnant that adheres to tooth surfaces. The body responds to this infection through tissue inflammation, an attempt to prevent the infection from spreading. Likewise, inflammation appears to be a similar response to changes in blood vessels afflicted by cardiovascular disease.
While inflammation is part of the body’s mechanism to heal traumatized tissue, if it becomes chronic it can actually have a damaging effect on the tissues intended to benefit. For patients with gum disease, chronic inflammation causes connective tissues to detach from teeth, leading eventually to tooth and bone loss. Similarly, inflammation damages the linings of blood vessels in cardiovascular disease patients.
Researchers want to know what role bacteria may also play in the progression of cardiovascular disease. Initial studies seem to indicate that proactively treating the gum disease by removing all plaque from oral surfaces in patients with both conditions does appear to improve the health of diseased blood vessel linings. Whether this could ultimately reduce the occurrence of heart attack or stroke still needs to be ascertained.
As we learn more about the possible connections between these two diseases, there’s hope it will lead to new advancements that could improve health outcomes for both. It may prove to be the case, then, that maintaining a healthy mouth promotes a healthy heart, and vice-versa.
